Cataract Surgery Guidelines Central Govt
1. Eye surgery should be done in an independent eye surgery home.
2. Only the patient should be examined in the camp and allowed to bring them to the hospital where the surgery is based for surgery.
3. All government institutions and private hospitals working under the National Blindness Control Program are required to enter into an agreement with the district level authorities as per the guidelines of the National Blindness Control Programme.
4. In no case eye surgery should not be done as a minor matter, vision is very important to live a simple life and hence success in eye surgery is very important to achieve normal vision.
5. Non-Government NGOs registered under National Blindness Control Program should be given permission by the District Surgeon and District Program Manager after checking their previous history and surgery to conduct eye check-up camps. The said organization should seek permission at least two weeks before the camp. Non-Governmental Organizations which perform eye surgery but are not registered should be supervised by District Surgeons and District Program Managers.
6. District Surgeon and District Program Manager should give permission to conduct eye surgery camp after evaluating the eye surgery home proposed by non-governmental organizations and the materials used to sterilize them, keeping in mind the instructions issued by the Central Government regarding the eye surgery home and other instructions.
7. Hospitals having infrastructure along with eye services viz
• Adequate number of beds in in-patient departments and independent or combined eye surgery house.
• Eye surgery home is in regular running condition.
• Hospitals should be allowed to conduct camps where eye surgery houses, both government and NGO, are being used with proper disinfection at all times.
• Eye camps should not be allowed in hospitals where the shift is crore or where the surgery house is closed for more than seven days.
8. The surgical team should consist of two Ophthalmologists with requisite experience, two Staff Nurses, one Ophthalmology Officer and one Surgery House Assistant.
9. An Ophthalmologist in the Surgery Team should have at least five years of experience working in Government or Non-Government sector or have a certificate of at least 500 surgeries after completing Post Graduate Degree or Diploma in Ophthalmology. The second Ophthalmologist should be a Post Graduate Diploma in Ophthalmology In any case the Ophthalmologist should work under the supervision of the Chief Ophthalmologist.
10. Preoperative examination is necessary to check the suitability of patients before cataract or glaucoma surgery.
11. Patients selected for cataract or glaucoma surgery should be given antibiotic eye drops as frequent eye drops as well as tablets like Diamox.
12. In a surgical camp not more than 20 to 30 eye surgeries should be performed by the Chief Ophthalmologist in a day.
13. A good quality and well-maintained microscope should be used for eye surgery and each eye surgeon should have a separate microscope. The main goal of all cataract surgery is to properly implant the IOL. Another ophthalmologist should take the eye surgery slowly and not rush it.
14. Full attention should be given to disinfecting the eye surgery room. It should be ensured that the equipment in the ophthalmic surgery room such as microscope, surgical trolley, clothing used by patients and surgical team, all surgical instruments and IOLs etc. are sterile. Under no circumstances should inferior sterilizing materials such as industrial spirit and acetone be used.
15. Surgical teams should ensure that they have sufficient supplies of surgical sets available for each patient to use separate materials to ensure proper sterilization throughout the day.
16. The supply company of all materials used in the pre-surgery camp must check expiry packing and cleanliness etc. It is important to shake the normal saline bottles used in surgery and carefully observe them to see if there are any impurities.
17. Cataract surgery should always be performed in a hospital OT with proper sterile care.
18. Proper bandage should be applied to the operated eye in the eye surgery house.
19. Every patient should be given proper treatment after eye surgery to avoid infection and other complications after eye surgery.
20. The Ophthalmologist along with his chammu should make regular examination of every patient on the first third and seventh day after eye surgery.
21. Future follow-up is necessary to regain full vision.
22. Photo ID of each patient should be preserved in cataract surgery record with address mentioned in contact number to facilitate early contact if required.
